The belay mistakes that keep showing up, decade after decade
Almost nobody drops a partner because they did something exotic. They drop a partner because of four or five errors that repeat across gyms, crags and countries with depressing consistency.

Ask a room of instructors what actually causes belay accidents and you will get the same short list every time. Not exotic failures. Not gear breaking. The same handful of human errors, repeating, in gyms and on granite and on limestone, across every country that has climbing in it.
That consistency is useful. It means the problem is knowable, and it means a small amount of attention pointed in the right direction removes most of the risk.
Losing the brake hand
This is the one. Every other error on this list is survivable if the brake hand stays on the rope, and most of them are fatal if it doesn't.
The brake hand comes off for ordinary reasons — to scratch, to swap hands during a long feed, to check a phone, to gesture at someone. The climber is fine at that moment, so nothing happens, so the habit gets reinforced. Then one day the climber falls during the two seconds the hand was elsewhere.
The fix is not vigilance. Vigilance decays. The fix is a technique in which the brake hand physically cannot leave — a proper slide-and-lock or pinch-and-slide feed, drilled until it is boring, so that letting go feels wrong rather than merely inadvisable.
The unfinished tie-in
Someone starts tying in, gets interrupted, and comes back to a figure-eight that looks complete because the first pass is through the harness. It is not complete. The follow-through has one strand where it needs two, or the tail is a centimetre long, or the rope went through only the leg loop.
Interruption is the whole mechanism. If you are stopped mid-knot — someone asks a question, a rope needs untangling — the correct response is to untie completely and start again, not to resume. Resuming assumes you correctly remember where you were, and human memory for motor sequences is not that reliable.
The device threaded wrong
Assisted-braking devices are excellent and they have created a new error class. Load strand and brake strand reversed produces a device that feels normal while paying out and does nothing at all when weighted. It is possible to lower someone smoothly on a backwards device right up until the moment there is real load.
Every one of these devices has a diagram moulded into the body showing which side takes the climber. Look at it. Then weight the system before the climber leaves the ground — have them hang on it, or pull hard on the load strand yourself. A backwards device announces itself instantly under load and never before it.
Lowering off the end of the rope
The route was longer than the rope. The climber is lowered, the rope end passes through the device, and the climber falls the remaining distance.
This one has two independent fixes and you should use both. Close the system — tie a knot in the free end, or tie into it, every single time, including on routes you have done before. And know the route length before leaving the ground, because rope lengths get trimmed after core shots and nobody updates the mental model.
Standing in the wrong place
Not lethal on its own, but it multiplies everything else. Too far from the wall and the belayer gets pulled forward and up on a fall, which introduces slack exactly when nobody wants it. Too far to one side and the climber swings into something. Directly under the first bolt with a heavy climber above and the belayer becomes a projectile.
The general position is close, slightly to the side of the fall line, with the rope running cleanly, and with the belayer's weight where an upward pull will be absorbed rather than resisted with surprise.
Slack management on lead
Too much slack means a longer fall than necessary and a higher chance of hitting a ledge. Too little means short-roping a climber at a clip, which is how people get pulled off. Neither is a rule you can write down and follow. It is attention — watching what the climber is doing, anticipating the clip, feeding fast and then taking it back.
A belayer looking at their phone cannot do this. A belayer talking over their shoulder cannot do this. The job is to watch the climber, and that is nearly the entire job.
The partner check people stopped doing
Buckle, knot, device, carabiner, closed system. It takes eight seconds. Almost everyone learns it and a large fraction quietly abandons it once they feel competent, which is precisely the point at which the errors above start to appear.
Do it out loud. Touch each item. The theatricality is the point — it converts a passive glance into an active check, and an active check is the only kind that catches anything.
