Heat Illness Builds Over Hours Not Minutes
Serious heat emergencies in the backcountry develop through a long approach rather than suddenly, and the early signs are easy to attribute to ordinary effort.

Heat illness in the American outdoors concentrates on long approaches, desert climbs and exposed river days. It develops slowly, which is precisely what makes it difficult to notice in yourself.
The body sheds heat through a limited set of routes
Heat leaves the body by radiation, convection, conduction and evaporation, and in hot conditions evaporation of sweat does nearly all the work.
That route depends on air being drier than the skin surface. In humid conditions, sweat runs off without evaporating and the cooling largely stops.
Which is why humid heat is more dangerous than the temperature alone suggests, and why heat indices exist to combine the two into a single warning.
Effort and clothing are the variables you control
Working muscles generate substantial internal heat, so pace is the largest lever a person has over their own thermal load on a hot approach.
Clothing that shades the skin while allowing air movement outperforms bare skin in direct sun, which reverses the instinct to remove layers.
Timing is the other lever: starting early enough to be off exposed terrain before the hottest hours is a scheduling decision made the night before.
The early signs look like ordinary fatigue
Heat exhaustion presents as heavy sweating, weakness, headache, nausea and lightheadedness, all of which a tired person attributes to the climb.
The change worth watching for in a partner is behavioral: irritability, stumbling, confusion or a decision that does not match how that person usually thinks.
Altered mental status separates a serious situation from a manageable one, and it is a signal to stop and cool rather than to push to the shade ahead.
Cooling and evacuation are the response
Getting the person out of the sun, removing excess clothing, wetting the skin and moving air across it are the standard field measures.
Anyone showing confusion or who does not improve needs emergency medical care, and the decision to call should be made early rather than after watching for an hour.

Water alone is not the whole answer
Sweat carries electrolytes as well as water, and prolonged heavy sweating replaced only with plain water can produce its own set of problems.
People with heavy sweat rates on multi-hour days generally manage this by eating salty food alongside drinking, which is the ordinary version of the same principle.
Individual needs vary widely with acclimatization, fitness and medication, so anyone with a medical condition affecting heat tolerance should have that conversation with a clinician before the trip.
