Hold-downs: breath, panic and the two-wave set
Being held under is where surfing gets genuinely serious. The physiology is manageable and the psychology is the harder half.

Most surfers, most of the time, are under water for a few seconds. Long enough to be unpleasant, short enough that nobody thinks about it. Then one day a wave holds you down and the few seconds keep going, and the experience is genuinely frightening in a way that nothing before it prepared you for.
Understanding what is happening makes a substantial difference to how it goes.
Your urge to breathe is not about oxygen
This is the single most useful physiological fact for anyone in the water. The desperate feeling that you must breathe now is driven mainly by rising carbon dioxide, not by falling oxygen. Your body detects CO₂ far more sensitively than it detects oxygen depletion.
Which means: when the urge arrives, you almost certainly still have usable oxygen. The alarm fires early. It is not lying, exactly — it just goes off well before the situation is actually critical.
Knowing that does not make the urge go away. It does let you meet it with something other than panic, and panic is the thing that turns a survivable hold-down into a bad one.
Panic is expensive
Thrashing consumes oxygen fast. Muscular effort under water burns through your reserve at several times the resting rate, and an elevated heart rate driven by fear does the same thing.
The person who relaxes and lets the wave finish with them surfaces comfortably. The person who fights from the first instant surfaces desperate, out of breath, and unready for the next wave — which is the actual danger, because it is usually the second or third wave that causes the injury.
So: go limp. Let it roll you. You cannot swim against the turbulence of a breaking wave and the attempt buys you nothing.
Protect your head
Arms up around the head, elbows out, as you go over. Most serious surfing injuries are impacts — with the sea bed, with reef, with your own board, or with someone else's. The hold-down itself rarely does the damage.
When you feel yourself being driven downward, expect the bottom. In shallow reef breaks, going down head-first is how people break necks. Try to land flat, or feet first, and cover.
Know which way is up
Disorientation is normal and it is worse when the water is aerated and dark. Two reliable references: buoyancy, and your leash.
Relax for a moment and you will start to rise — that is up. Or find your leash and follow it, because the board floats. Both take a couple of seconds and both are more reliable than your inner ear after a spin cycle.
Surface ready for the next one
Come up with hands over your head. Take one breath — a full one, not a gasp — and immediately look outside to see what is coming.
The two-wave hold-down is where things get serious: you surface, get half a breath, and the next wave lands. If you know that is likely, the correct move is often to take a full breath and duck deliberately under the second wave rather than being caught on the surface by it.
This is why breath capacity matters less than breath discipline. One calm full breath beats three panicked half breaths.
Training, with a serious caveat
Breath-hold training genuinely helps: static holds, CO₂ tolerance tables, and general aerobic fitness all extend comfortable time under water and, more importantly, reduce the panic response by making the sensation familiar.
Never do breath-hold training in water alone. Shallow water blackout kills people every year, including strong and experienced swimmers, and it happens without warning — no struggle, no signal, just unconsciousness. Any in-water breath work needs a dedicated partner whose only job is watching you, within arm's reach.
Dry training — static holds lying down, on land, with someone present — carries none of that risk and delivers most of the CO₂ tolerance benefit.
And never hyperventilate before a breath hold. It suppresses the CO₂ alarm without adding meaningful oxygen, which means you can pass out before you ever feel the urge to breathe. This is the exact mechanism of shallow water blackout.
The honest framing
Every step up in wave size is a step up in hold-down duration and consequence. Go up in increments, in water you know, with people around who are paying attention.
The surfers who handle big water calmly did not acquire that by being brave. They acquired it by being held under many times in progressively bigger surf until the sensation stopped meaning emergency.
