The Anatomy Of An Accident Chain
Serious incidents are almost never caused by a single failure, but by a sequence of small compromises that each seemed reasonable at the time.

Accident reports rarely describe one mistake. They describe a sequence, each step small enough to pass unnoticed, which collectively removed every margin the party had.
Single causes are usually an illusion
A report might record that a rope was too short for the rappel. That fact is true and incomplete, because a functioning system would have caught it several ways.
Knots in the rope ends, a check by the second person, and knowing the descent length are independent defences. The accident required all of them to be absent.
Focusing on the final failure produces a lesson that is too narrow to transfer. The useful question is why the earlier defences were not in place.
How the chain typically forms
The links tend to look mundane: a late start, an unfamiliar partner, equipment borrowed at the trailhead, a forecast checked two days ago, a route description skimmed.
Each is individually survivable and individually reasonable. Their effect is cumulative, because each one removes a margin that would otherwise have absorbed the next problem.
By the time the visible incident occurs, the party has usually been operating without reserves for some hours, which is why the outcome escalates so quickly.
Why time pressure recurs
The most frequently recurring link is being behind schedule. Time pressure degrades decisions directly, because the options that take longer become psychologically unavailable.
It also compounds itself. A rushed rappel takes longer when it goes wrong, and a party that skips a rest makes poorer choices an hour later.
This is the mechanism behind alpine starts and turnaround times. Both exist to keep the party out of the situation where haste starts making the decisions.
Near misses are the same chain, interrupted
An event where nothing was damaged has the same structure as one where somebody was. The difference is often where a rock landed, which is not a skill.
Treating near misses as luck rather than as success is what makes them useful. They are a free look at a chain that has not yet completed.
Groups that discuss them without assigning blame surface the early links, which are the ones anybody can still change on the next trip.
Where the chain is easiest to break
Intervention is cheapest at the start. Deciding not to go, changing the objective, or resolving equipment problems at home costs nothing compared with the same decision at altitude.
The later links are hard to break because the party is committed, tired and invested, which is exactly when judgement is least reliable.
Reading published incident analyses from national rescue and avalanche organisations is the most direct way to see how these sequences form, since they document the early links that participants rarely notice themselves.
